    That piece of paper found every morning in the workshop

    In many factories, the day begins the same way: a small group gathers in front of the workshop's tracking board. There you find the indicators, the workload plan… and often a piece of paper slipped into a plastic sleeve.

    Sometimes, if the night shift is well organised, there is even a small pre-filled template stuck to the wall:

    "Who? What? Where? When? How? Why?"

    That morning, on line 3, a non-conformance was reported:

    • Who: Team B, operator A

    • What: Crimping defect

    • Where: Station 5

    • When: 15:42

    • How: Visual defect

    • Why: Hypothesis — pliers to be recalibrated

    A well-established reflex: the 5Ws and 2Hs (or 5W1H) method was used.

    But the problem is not being addressed.

    No action has been initiated.

    The morning team leader does not know if the defect has been resolved or not.

    And above all, this is not the first time this defect has been reported.

    5W1H method: a simple and effective method for structuring a finding

    The 5W1H method relies on 6 essential questions:

    Where did it happen?

    Locate the action in space

    Originating from journalism and then popularised in industry, this method is now a shop-floor reflex in quality, safety, production, and maintenance processes.

    It is used in 8D, QRQC, problem-solving methodologies, or non-conformance reports.

    Its strengths: speed, clarity, accessibility

    5W1H is highly valued because it is:

    • Simple to understand and apply

    • Usable by all levels, from the shop floor to management

    • Adaptable to any sector

    • Ideal for initiating a discussion or analysis

    It often helps to avoid quick judgements or rushed decisions: you start by observing, questioning, and formalising.

    It is a method that brings clarity.

    But not necessarily action.

    The limitations (and pitfalls) observed on the shop floor

    In the reality of factories, 5W1H sheets sometimes accumulate without any follow-up:

    • No subsequent action initiated

    • No link between the findings and the resolution

    • No tracking or feedback to the teams

    • Unidentified recurrences

    • Misplaced documents, forgotten files

    The method then becomes an administrative formality.

    And this demotivates the teams, who see no value in reporting non-conformances.

    👉 Tip:

    A 5W1H sheet has value only if it triggers an action, a discussion, or a decision.

    It is not an end in itself, but a trigger.

    Using 5W1H effectively: our advice

    1. Avoid vague or generic answers

      👉 Replace "part defect" with "excessive play on rear chassis, fasteners not tightened".


    2. Display a well-completed 5W1H example in the workshop

      👉 Visually show the expected standard.


    3. Create a clear channel for the next steps

      👉 Non-conformance sheet? Action to assign? QRQC meeting? It must be obvious.


    4. Centralise 5W1H data in a living medium

      👉 A shared log, or better, a dedicated tool.

    Going further: structuring and capitalising

    While 5W1H allows you to lay the foundations, it is not enough to build a high-performing non-conformance management system.

    You need to be able to:

    • Track the history of findings

    • Link observations to actions

    • Identify recurring causes

    • Have a multi-site or multi-process vision

    • Automate reminders, summaries, and indicators
